Skull positioning

Mandible Axiolateral and Oblique Axiolateral is a radiography positioning projection of the Skull. Axiolateral & oblique axiolateral mandible. CR 25° cephalad. The centering point is located mandibular angle region. head rotated 30° from true lateral.. The central ray is cr angled 25° cephalad, entering through the mandibular region opposite the side of interest.. Image-quality criteria include 1 inch beyond lateral mandibular area visualized., tmj articulation or mandibular angle clearly outlined.. Standard exposure ranges from 70 to 80 kVp, 12 to 20 mAs, at an SID of 40 inches (102 cm).

Anatomy demonstrated

  • Mandibular body in axial perspective.
  • Mandibular angle and ascending ramus.
  • TMJ region and condyle in projected area.
  • Lateral aspect of mandible without foreshortening.
  • 1 inch region beyond lateral border of angle.
  • Body of mandible and alveolar process.

Patient preparation

  • Verify patient identity.
  • Remove glasses, earrings, dentures, hairpins.
  • Position patient upright or prone in true lateral position.
  • For oblique variant: rotate head 30 degrees from true lateral.
  • Mid-sagittal plane parallel to IR (or at 30-degree angle for oblique).
  • Suspend respiration during exposure.

Position & centering point

Mandibular angle region. Head rotated 30° from true lateral.

Central ray

CR angled 25° cephalad, entering through the mandibular region opposite the side of interest.

Image-quality criteria

  • TMJ articulation or mandibular angle clearly outlined.
  • Body of mandible visualized in axial projection.
  • Ramus and angle demonstrated without foreshortening.
  • Minimal overlap of mandible and surrounding facial bones.
  • Detail of mandibular structure at region of interest.

Common errors / ARRT traps

  1. 1 CR not angled 25 degrees: foreshortening of targeted region.
  2. 2 Head rotation incorrect: off-center projection of mandible.
  3. 3 Rotation: mandibular body and angle not clearly visualized.
  4. 4 Tilt: asymmetrical appearance of mandibular region.
  5. 5 CR not horizontal: anatomic distortion.

  • Mandibular angle fracture assessment.
  • TMJ region evaluation with mandibular body.
  • Specific mandibular region trauma.
  • Evaluation of mandibular body pathology.
  • Pre- or post-operative follow-up of mandibular region.
